In room 2, I observed rubber cement stored unlocked in the cabinet over the sink. The rubber cement was discarded during the visit. In room 1, I observed Clorox wipes stored unlocked in the cabinet over the sink. In room 1, I observed an emergency medication stored less than 5 feet, not out of the reach of the children. The cabinet over the sink was locked and the medication was stored 5 feet high, out of the reach of the children.

The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with parents of currently enrolled children 0-5 within 30 days of adopting the policy. I did not observe signed acknowledgement the SBSAHT policy was reviewed in one child's file.

I did not observe signed acknowledgment in two staff files that the SBSAHT policy was reviewed with staff prior to providing care. During the visit, the two staff signed acknowledgement review and receipt of the policy.
