The following staff were observed in a child care role at the facility. Staff #3 did not have on file a health assessment. Staff #4 did not have on file the results of a tuberculosis screening.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem. A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: An unannounced inspection occurred at the facility. A review of staff files was conducted. Regulation requires staff to participate in pediatric first aid/CPR prior to their current certification expiring. The following staff did not participate in pediatric first aid/CPR prior to their current certification expiring: Staff #1 and #2. The current certification for these staff expired February 2026 and these staff did not take training again until 3/10/26.

Correction Required: Competence is the completion of training by a professional in the field of first-aid and cardiopulmonary resuscitation (CPR). All staff persons shall renew their certification in pediatric first aid and pediatric cardiopulmonary resuscitation (CPR) on or before the expiration of the most current certification.

The CPSL requires staff to have a completed NSOR on file prior to working in a child care role at the facility. Staff # 3 did not receive an NSOR until 5/20/26 which was after the staff's date of hire. Regulation requires staff to have on file prior to working in child care a fully completed signed and dated disclosure statement. Staff #3 and #4 did not complete a disclosure statement until 5/12/26 which was after both staff's date of hire.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Description: Small Toys and Objects

Noncompliance Area: A renewal inspection occurred at the facility on 8/14/25. A physical site review was conducted. In the large main room, in the diaper changing table drawers, plastic bags and wipes in plastic bags were found. Also in the children's cubbies, wipes in plastic bags were observed along with diapers in plastic bags. Toddlers were observed receiving care during this inspection. The wipes indicated the need to be kept out of reach of children due to plastic container.

Correction Required: Toys and objects with a diameter of less than 1 inch, objects with removable parts that have a diameter of less than 1 inch, plastic bags and styrofoam objects may not be accessible to children who are still placing objects in their mouths.

Noncompliance Area: A renewal inspection occurred at the facility on 8/14/25. A physical site review was conducted. In the left side room, a changing table was observed with a lower broken drawer.

Correction Required: Furniture must be durable, safe, easily cleaned and appropriate for the child's size, age and special needs.

Description: Supervised at all times /Staff assigned to specific children

Noncompliance Area: CONTINUED NONCOMPLIANCE An unannounced inspection occurred at the facility 6/9/25. A physical site review was conducted. In the 3rd large room from the entrance, a preschool age girl was observed unsupervised. It is unknown how long this girl had been in this room. At this same time, Staff #6 appeared to be looking for a missing child. Certification Representative asked Staff #6 if they were missing a child and Staff #6 admitted they were. This child was reunited with their group in the 3rd large room from the entrance with Staff #5. On a second occasion, Certification Representative was walking through the facility with the director and at that time, another preschool age child was found alone and unsupervised in the same section of the building. It is also unknown how long this child had been gone from Staff #5, as Staff #5 did not appear to be looking for the missing child. The director reunited this child with Staff #5, who appeared unaware that the child was missing. An acceptable plan of correction was received on 7/16/25. A renewal inspection occurred on 8/14/25. A directed plan was given to the facility, and an acceptable plan of correction was given that the facility would arrange for all facility staff hired as of the date of the training to take PQAS-approved training for supervision. As of the inspection on 8/14/25, this training has not been taken.

Correction Required: Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. Each staff person shall be assigned the responsibility for supervision of specific children. The staff person shall know the names and whereabouts of the children in his assigned group. The staff person shall be physically present with the children in his group on the facility premises and on facility excursions off the facility premises. TIERED LIS: 1. Children must be supervised at all times. This portion of the plan shall have an immediate correction date. 2. The legal entity must arrange for all facility staff hired as of the date of the training to receive a minimum of three hours of in-person, PQAS-approved training, regarding supervision and appropriate ratios of children. The legal entity must receive DHS approval of the training content prior to scheduling the training. To show proof of the training, the legal entity will submit the staff attendance/sign-in sheet to the Regional Office after completion of the training. The operator shall provide a date for when this training will be completed.

Noncompliance Area: A renewal inspection occurred at the facility on 8/14/25. A physical site review was conducted. In the large main room, a plastic bag was observed hanging from the diaper changing table. Inside the plastic bag was a used diaper and wipes. This bag with dirty diaper and wipes as not immediately discarded and was accessible to children.

Correction Required: If disposable diapers are provided by a parent or by a facility, a soiled diaper shall be discarded by immediately placing the diaper into a plastic-lined, hands-free covered can.

An acceptable plan of correction was received 7/16/25. A renewal inspection occurred at the facility on 8/14/25. Staff #1 and #7 were working together at the time of this inspection. See the code sheet for the date of hire. Staff person #7 has not completed the following required pre-service training, is within 90 days of their date of hire date but was observed working unsupervised: pediatric first aid/CPR. Staff #1 had pediatric first aid training that expired on 5/3/25. Per regulation, these staff were not supervised by an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations.State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required training has been completed, staff persons #1, #5 and #7 must be supervised, when interacting with children, by an AGS who has completed all preservice trainings and has all qualifications to care for children unsupervised. If there are no staff available to supervise staff persons #1, #5 and #7, staff persons #1, #5 and #7 may not work in a child-care position at the facility.

Noncompliance Area: CONTINUED NONCOMPLIANCE An unannounced inspection occurred at the facility on 6/9/25. A review of staff files was conducted. See the code sheet for the date of hire. Staff #5 was not hired correctly as a provisional hire per the CPSL. Staff #5 was observed working unsupervised with children and the file did not contain the NSOR clearance or documentation of applying for the state police clearance as required by the CPSL to be hired as a provisional hire. As a provisional hire, Staff #5 was also required to be supervised, but Staff #5 was working unsupervised with children at the time of this inspection. An acceptable plan of correction was received 7/16/25. A renewal inspection occurred on 8/14/25. A directed plan was given to the facility, and an acceptable plan of correction was given that the facility would arrange for the legal entity, director and anyone responsible for maintaining staff files did not participate in Existing Provider Training. As of the inspection on 8/14/25, this training has not been taken.

Staff #2 was unable to be appropriately qualified to work in a child care position as the education from Brazil was not sent for equivalency. An acceptable plan of correction was received 7/16/25. A renewal inspection occurred at the facility on 8/14/25. A review of staff files was conducted. The file of Staff #2 contained education from Brazil. Staff #2 was unable to be appropriately qualified to work in a child care position as the education from Brazil was still not sent for equivalency.

Correction Required: A staff person or a substitute staff person shall meet one of the applicable staff qualifications for the position in which the person is performing. A facility person's record shall include verification of child care experience, education and training prior to service at the facility.

Description: Locked or inaccessible

Noncompliance Area: CONTINUED NONCOMPLIANCE An unannounced inspection occurred at the facility on 6/9/25. A physical site review was conducted. In the large main space, a bottle of A&D ointment was found in a child's cubby that was accessible to children, but the bottle indicated the need to be inaccessible to children. Children were observed receiving care in this area at the time of the inspection. In both rear bathrooms of the building, the cabinets were unlocked and contained Lysol and soaps. These bathrooms were observed being used by children at the time of the inspection. These items indicated the need to be inaccessible to children. The gate to the kitchen was left open and accessible to children were several disinfectants, paint, and cleaners. These items indicated the need to be inaccessible to children. Children were observed receiving are in the room directly attached to the kitchen's open gate. A door labeled 'Employees Only' was left unlocked. In this room the following toxic items were found: cleaner, bleach, floor stripper, laundry detergent and a mop bucket with dirty water and cleaner (water was gray and smelled of cleaner). Children were observed passing by this unlocked door that contained toxic item that the labels read to keep out of reach of children. An acceptable plan of correction was received 7/16/25. A renewal inspection occurred at the facility on 8/14/25. A physical site review was conducted. In the large main space, numerous bottles/containers of diaper rash creams were accessible in the diaper changing drawer and in the children's cubbies. In the small left room, diaper creams were also accessible in the diaper changing drawers. Children were observed receiving care in these rooms and the items found indicated the need to be kept out of the reach of children. These items were discussed with the director at the time of the inspection.

Correction Required: Cleaning materials and other toxic materials shall be kept in an area or container that is locked or made inaccessible to children.
