Noncompliance Area: According to CPSL announcement issued 9/18/2018, all staff hired prior to 9/30/2019 are required to have an NSOR verification on file no later than 7/1/20. This update also eliminated the 90 day provisional hire period for new staff unless the facility obtained a waiver. The NSOR verification for staff # 2, 3 and 4, all hired prior to 9/30/19 are dated 9/3/20. Staff person # 1 was hired on 12/30/2019 and their NSOR verification is dated 9/3/20. Without a provisional hire waiver on file, the NSOR verification was required at the time of hire.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: The pink slide is not secured to the plastic climber in the grassy play area; the landing area of this climber is cracked causing a pinch point. There are exposed bolts on the fence in the outdoor play area.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Noncompliance Area: During the renewal inspection, cert rep observed staff person # 1 change a child's diaper. The child's hands were not washed after the diaper changing and staff person # 1 used hand sanitizer instead of hand washing with soap and water.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered. A facility person shall wash his hands before meals and snacks, and after toileting and after diapering a child.

Noncompliance Area: There is no written consent for the administration of minor first aid procedures or consent for emergency medical care on file for child # 1.

Correction Required: A child's record shall contain signed parental consent for emergency medical care for the child. Written consent is required prior to admission. Noncompliance Area: More than 12 months passed between fire safety training for all staff members. Fire safety training certificates were dated 8/7/2018 and then 9/4/19.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent.

Noncompliance Area: The foot pedal on the trash can in the child care space is broken off leaving a metal bar exposed.

Noncompliance Area: There is a broken tile on the floor outside of the bathroom in the child care space. The baseboard molding on the wall behind the red table is not secured to the wall.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: There is a plastic play house in the outdoor space with a broken seat inside making a sharp edge and pinch point.
