Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please submit the specifications for the climbing structure on the playground as soon as possible. ***The monkey bars are not approved for use by children under the age of 5. 4 of 4 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Children's Records must be complete with all required information. 2. Medications and labels on over-the-counter medications. 3.
