Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 11/19/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please submit the specifications for the climbing structure on the playground as soon as possible. ***The monkey bars are not approved for use by children under the age of 5. 4 of 4 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The following was discussed but not limited to: 1. Children's Records must be complete with all required information. 2. Medications and labels on over-the-counter medications. 3. The equipment on the playground must be age-appropriate.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 11/26/2024, and are subject to changes pending programmatic review. Compliance Officer #1: Tricia Tartaglio Compliance Officer #2: Chloe-James Rossi The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but is not limited to: 1. Ensure all outdoor activity areas have a shade structure that provides enough shade for each enrolled child occupying the outdoor activity area. 2. Ensure all food waste containers have a lid. 3. Ensure the irrigation pipe does not pose a tripping hazard to children. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 12/1/2023, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Survey link was sent to the Director during the inspection.
