Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 8/19/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The Emergency Disaster Contact Form was completed at the time of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The fingerprint clearance cards for 1 of 1 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *The new rule set and the new forms. *Ensure the weekly lesson plan has the daily activities the children will be doing, and your lesson plan includes a date.

Initial Comments: There were no deficiencies observed at the time of the monitoring inspection conducted on 7/22/2025 1 of 1 fingerprint clearance card was verified to be valid through the DPS website during the time of the inspection. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The following was discussed, but not limited to: 1. Staff file documentation. 3. First aid kit components. 4. Screen time documentation. 5. Water dispenser maintenance and accessibility to enrolled children.

Initial Comments: The following deficiencies were observed at the time of the Modification inspection conducted on 6/18/2025, and are subject to changes pending programmatic review. A full inspection was not conducted at this time. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. Notifying parents and documenting injuries. 14. Illness and infestation log. 15. Medication procedures. 16. Transportation and Field Trip procedures. 17. Notify the Department of closure for any days. 18. Interview rights of staff and children.

Initial Comments: The Compliance Inspection due at this time could not be conducted due to the temporary closure of the facility. The temporary closure was confirmed on 9/27/2023 by a phone call to Principal Fuad. Compliance Officer is Heather Bauer.
