Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 5/8/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to a staff at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***A staff was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 5 of 5 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed at the time of the inspection. The Emergency Disaster Plan update form was emailed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Toilet room ceiling vents must be maintained in a clean condition. 2. Caulking around toilets and sinks must be maintained in a clean condition. 3. Children's Record cards must be complete and have immunizations attached. 4. The current weekly menu must be posted on the main posting board. 5. Staff files must be complete with the DCS submittal results.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 5/9/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Please also submit the current Fire Marshall inspection, when completed since the fire Inspection expired on 5/11/2024. The following was discussed, but not limited to: *Start date versus hire date/date of employment. Fingerprint clearance cards are required to be issued prior to the staff’s date of employment, *Reminder to update the name of the Director in the Licensing Portal to the current director, and *Reminder that the license anniversary fee is due in July 2024 and that the Empower Program is no longer subsidizing the fee. There were 3 staff files reviewed. 3 of the 3 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment was completed at the time of the inspection.
