The Kid Zone Enrichment Program is a dynamic community response to changing lifestyles and needs of families in Tempe, Arizona. Our goal is to address the increased number of children needing a safe, enriching place to be in out-of-school time. Kid Zone operates both before and after school programs at 19 Tempe and Kyrene Schools. Our program is for children in the grades K-5 at 15 of our schools and K-8 at 3 of our schools (Curry, Rover, Ward). We also offer a Pre-School Program for 2 year olds - Pre-K's.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 3/4/2026 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. A Plan of Correction will not be accepted at this time. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 4 of 4 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Emergency Disaster Plan update form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation link to the Provider. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Torn and frayed edges on blue mats. 2. Staff in school must have recent school documents showing they are currently enrolled in school.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 3/7/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***Please submit documentation showing the fire alarm panel is in operable mode as soon as it has been fixed. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was completed at the time of the inspection. The Emergency Disaster Plan update form was completed at the time of the inspection. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Children's sign in and out sheets must have a first initial and full last name. 2. Ensure the required postings are posted in each activity area. 3. Ensure the playground is maintained free from hazards and has the required 6 inches of resilient surface in all fall zones.

Initial Comments: The following deficiencies were found at the time of the Compliance Inspection conducted on 3/28/2024, and are subject to changes pending programmatic review. Name of Compliance Officer: Jennifer Flicker The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. **Please submit the Current Fire Marshall inspection when completed. There were 2 staff files reviewed. 2 of the 2 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Assessment link was emailed to the Director at the time of the inspection.
