Findings: Based on Surveyor's observation during the inspection of the diaper changing area in the 3 year olds room, the soiled diaper container and the soiled clothing container were not equipped with lids. The lids were laying next to the containers. The soiled diaper container was not equipped with a liner.

Findings: Based on Surveyor's observation during nap time, the following was noted: *One year old room- 6 occupied mats were spaced only 12-14 inches apart; * Two year old room- An occupied mat had a pathway of only 12 inches wide and was obstructed by a stack of chairs. Two occupied mats passageways were obstructed by other mats; and *3 year old room- 3 occupied mats was obstructed by a bookshelf, a sofa and other children's mats.

The Co-Director stated she usually is in the front office area but was in the rear of the building and did not hear the doorbell and the door was locked. The Surveyor waited outside for 3 minutes before a staff member noticed and allowed the Surveyor entry.

Findings: Based on Surveyor's inspection of the diapering area in the one year old room, a sponge was stored in the diapering sink.

Findings: Based on the Surveyor's observation, the facility Outdoor Playground area was not maintained in a clean condition due to the following: 1) The sidewalk was covered in piles of leaves, branches and dirt; 2) A 3 foot long dead branch was observed under the tree; 3) A 2 foot wide weed was growing near the sidewalk; 4) The top of the white table was covered in a layer of dirt and brown smudges; and 5) The storage container and the toys inside it were covered in a layer of dirt, sand and brown smudges.

Findings: Based on the Surveyor's observation, the toilet paper roll in the 3 year old room was not mounted and was laying on top of the toilet tank. The toilet paper mounting bracket was broken off.

Findings: Based on the Surveyor's observation, the sand measured the following depths: *Less than 1/2 of the required 6 inches under the monkey bars and under the swings; and *Less than 3 of the required 6 inches under the yellow climbing structure. The sand under the entire monkey bar structure and swing set was extremely hard and compacted. The Surveyor and the Director used a screwdriver to scrap the area to ensure that it was not concrete.
