The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. ***The Director was reminded of the license expiration date and to log on to the portal to pay the fees prior to the license expiring. 2 of 2 Fingerprint Clearance cards reviewed were valid via the DPS website search. The Empower self-evaluation was emailed to the facility. The Emergency Disaster Plan update form was emailed to the facility. The DES Group Size requirements were observed to be compliant at the time of the inspection. The following was discussed but not limited to: 1. Extension cords are not permitted to be used. 2.
