| Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 1/22/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. The DES Contact Group size was in compliance at the time of the inspection. The fingerprint clearance cards for 6 of 6 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *The wood must be monitored on the raised sensory table located on the Younger playground. *Ensure the starting and ending dates on the Medication forms are listed with month, day, and year. *Maintain daily documentation of the presence of an enrolled child in an activity area that includes a method to account for any temporary absence from the activity area. *Screen time shall be listed in minutes on a lesson plan.
