| Initial Comments: The following deficiencies were observed at the Initial Inspection conducted on 11/12/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 16 of 17 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1) New rules, effective 8/3/2025, 2) Staff files items that need to be updated due to the change in ownership: TB attestation, DPS affidavit, DCS background check, Date of employment and Orientation training, and 3) Crib spacing requirements when side-by-side cribs are occupied and the cribs are not placed plexiglass to plexiglass. Please submit the corrected architect memo to include the dimensions and square footage of each outdoor activity area. | |||
