| All equipment and furnishings were not in good repair. The mat under the swing was damaged and had a hole in the middle of it. Although they had mulch and raked it so that they met the 6 inches in depth to ensure protective surfacing was sufficient the mat was not removed even thought it was not in good repair. | |||

| Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. One child, that had an inhaler, did not have a medical action plan. The child had information regarding medication administration but no action plan to identify and treat any asthmatic symptoms. I explained that the plan needed to go into the child's file, in the Ready to Go file (EPR plan) and best practice would be in the classroom accessible to the staff so that they can consult the form to determine if there is a medical need for medication and can follow the instructions forthwith. | |||
