A safe indoor and outdoor environment was not provided for the children. The mulch on both playgrounds that are being used had the required amount of mulch, but it was compacted and rotting therefore not providing the required protective surfacing that would allow for the correct cushioning which was causing an unsafe environment. Some of the mats at the ends of the slides were not positioned or were torn therefore could not provide the appropriate surfacing at the exits of each slide causing an unsafe environment. | |||
