When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. In the three classrooms serving Preschool Children did not have materials in sufficient quantity for three children to interact with materials that would make an activity in the Creative Dramatics/home living Center, two of the classrooms did not have sufficient quantity for three children to interact with materials that would make an activity in the Language or Art Centers. It was reported that materials had been ordered but the violation was cited on 07/`7/2023 and the materials order was not placed until 07/28/2023. This is a repeat violation.

A safe indoor and outdoor environment was not provided for the children. As you enter into the preschool classroom playground gate there is exposed cement that is uneven and could be a tripping hazard for children, staff and parents. It was reported that it has been schedule for repair. This is a repeat violation.

Documentation of staff's on-going training was not on file and/or was not current. Of the eleven (11) staff files reviewed there were three staff who had not completed the required health and safety training within the required time frame. At today's visit it was reported that the additional training hours had not been completed. This is a repeat violation.

The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. There was an EPR plan in the Ready to Go binder, but it didn't have the required detailed information documented on the pages. It appeared to be printed out without finishing the document. It was in DRAFT form. This plan was to be completed in its entirety within four months of the Administrator completing the EPR training which was in 2019. At today's visit it was reported that the ERP plan had not yet been updated. This is a repeat violation.

The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. On the staff and training worksheet it is documented that the staff was trained on the EPR plan for all staff, but the plan was not complete. At today's visit it was reported that the EPR plan and Ready to Go binder had not been updated therefore no training has occurred. This is a repeat violation.

Other surfacing materials not identified in rule were used that did not meet the ASTM standards, were not installed, maintained, and/or replaced according to manufacturer's instructions. The poured rubber protective surfacing materials has some areas that were beginning to show wear and were torn or ripped. Some of the areas had been repaired but there were two areas on the Toddler Playground and two areas on the School Age playground that needed repair. Based on the tearing of the surfacing material it was not maintained according to manufactures instructions. This is a repeat violation.

Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Today the arrival and departure records for one child in Group 1 and one child in Group 4 was not completed. This is a repeat violation.

Caregiver did not respond at the earliest opportunity to an infant or toddler’s physical and emotional needs. In Space 1, where children under twelve months were located, the children were crying throughout the entire observation. One child was asleep in a highchair and one child ate a left-over goldfish from off the floor. In Space 3, where children ages 1 years of age were located, the caregiver had her back turned to the children and they were pushing an adult child around the classroom and were knocking into the other children.

When screen time was provided to school-aged children, it was not offered as a free-choice activity; not used to meet a developmental goal; was not limited to 30 minutes per day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. In Space 10, where the school age children were located, it was observed that one child pulled his phone out of his pocket and played on it and then put it back into his pocket. This screen time interaction was not documented on a cumulative log.

A safe indoor and outdoor environment was not provided for the children. In space 5 there was a radio that had a cord hanging down and plugged into the outlet. This cord if pulled would have made the radio fall off of the window ledge or could be used to wrap around a child's neck.

Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. The diaper creams, sunscreen, but spray, and the Benadryl was not kept 5 feet up (for topic items) nor in locked storage (for Benadryl) as they were in the office in a storage box on floor level or on the top of the cabinet which was less than 3 feet from the floor.

Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In the office area which is usually kept locked there was left over medications that did not have permission slips completed including diaper creams, bug sprays, sunscreens, benyadryl and epi pens.

Center staff did not comply with the safe sleep policy. The facilities safe sleep policy states that children will be placed in their crib on their backs to sleep and a child was in a highchair slumped over in the chair sleeping.

I have used this provider for more than 6 months

I don’t even know how I can start to express how much I love love love childrens lighthouse Waxhaw. It has been a sanity and life saver for me. My babyboy has been going there regularly since he was a tiny little infant. Their directors are so friendly and make you feel at home. They now have a new director, Tatyana, who is so welcoming and so friendly. The teachers are so sweet, so involved and so caring. Soon as my now 18 months old toddler gets to his classroom he hugs his teachers. That shows me how much he loves them. All his teachers are just great.

I was not the kind of mom that easily wanted to drop my baby at a daycare. But once I visited children’s lighthouse, all my anxiety was put to rest. They have cameras in the rooms so you can check on your little one online anytime anywhere once you register. They also give you a daily log showing what your little one has been up to all day. What they ate and how much, diaper changes and their mood that day.

They have a fantastic play area outside and a play gym (No boredom for the children at all). Trust me if you have a toddler, they would probably rather be at a cool place like this than stay home. The children are fed a great variety of meals so you don’t even have to pack anything. They have a structured program and teach a lot to the children.
