Caregiver did not respond at the earliest opportunity to an infant or toddler’s physical and emotional needs. In the classroom serving Toddlers the two staff were observed in the outdoor learning environment where a child was very upset over a toy. The staff were ignoring her fussing and did not respond to the child nor provide the child with the skills needed to learn to self-regulate. The child was emotional and there was no response until it was brought to their attention by the Administrator.

Children were not adequately supervised at all times. On Monday April 19, 2023 a one-year-old child was left outside for approx. twenty-five (25) minutes. The staff member did not know the child was missing nor did she have any idea where she was for a period of time.

Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. It was reported by the Chef that some food substitutions have been made to the menu but they have not documented these changes.

All openings to the outer air were not protected against the entrance of flying insects. The staff reported that they had been propping the door open in order to air out the room due to the rooms recently being painted.

The discipline policy was not followed. One staff person, LS, placed a child in bathroom with the door shut and due to the motion sensor lighting, while the child reportedly sat still the light was off as a form of discipline. This violated the facilities discipline policy.
