Initial Comments: The following deficiencies were observed at the time of the Initial Monitoring Inspection conducted on 8/25/2025, and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Designee at the time of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website. The Empower Survey was emailed to the provider at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. The following items were discussed but not limited to: 1. Ensure diaper changing bins and soiled clothing bins are located in diaper areas. 2. Ensure capacity and evacuation maps are posted in each activity area. 3. Ensure fans are mounted. 4. Ensure brooms and dustpans are inaccessible to enrolled children. 5. Ensure classroom rosters are maintained at all times. 6. Ensure plungers are removed from restrooms. 7. Ensure all exhaust fans are free from dust and debris. 8: Ensure all medication authorization forms are complete. 9. Ensure all staff know which children have medications.

Initial Comments: The following deficiencies were observed at the time of the Initial Inspection conducted on 4/17/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. Please submit pictures of the fence when the gaps have been covered/fixed. 17 of 17 Fingerprint Clearance cards reviewed were valid via a DPS website search. The following items were discussed: 1. Staff attendance records 2. Monthly fire drills 3. Notification of pesticides 4. Children Record cards 5. Children's Immunization Records 6. Children's sign in and out sheets 7. Classroom rosters 8. Child abuse and neglect 9. Accessible drinking water in activity areas 10. Posting & specific details on lesson plans 11. Children with special needs 12. Specific items on the menus 13. Discipline and guidance 14. Spacing of napping children 15. Updated written Accident, Evacuation and Emergency plans 16. Notifying parents and documenting injuries 17. Illness and infestation log 18. Medication storage and forms 19. Field trip form 20. Interview rights (staff & children alone) 21. The required postings must be posted in each activity area. 22. Chemicals, toxins and hand sanitizers must be inaccessible to enrolled children. 23. Blind cords must be inaccessible to enrolled children. 24. Fans must be mounted and inaccessible to enrolled children. 25. Outlets must be covered with a safety plug. 26. Menu items must be specific.
