In the E4 Classroom the staff-to-children ratio was 1:16 rather than 2:16 as required for 4-year-old children. Based on staff interview, facility documentation, and the Surveyors' observations, in the hall area, the Surveyors observed an enrolled child with two individuals that were not staff of the facility. The staff-to-children ratio was 0:1, rather than the required ratio of 1:1. The Director stated that the two individuals that were with the enrolled child were from the MelMed center and she did not have staff files for these individuals.

In the Voyager 3's and Voyager 4's classroom, there were diapers in a plastic bag on the diaper changing surface. In Voyager 7's classroom, there were keys, a container lid, and a plastic bin on the diaper changing surface.

Toddler 3 Classroom: *The toilet tank lid was missing. Preschool 2 Classroom: *There was peeling paint with exposed wood located on the West wall. Preschool 3 Classroom: *There was peeling paint with exposed wood located under the bulletin board on the East wall. Preschool 5 Toilet Room: *The toilet seat was loose. * The caulking at the back of sink was loose and the sink was easily moved when lightly pushed. School-age Classroom: *There was peeling laminate with exposed particle board located on the South wall. *There was a popcorn machine located on a low shelf near the South wall that tipped easily when lightly touched. *There was cracked tile with exposed drywall and a protruding nail located on the wall above the toilet.
