Initial Comments: The following deficiencies were observed at the Compliance Inspection conducted on 3/27/26, and are subject to changes pending programmatic review. The Plan of Corrections is requested to be submitted in the LMS portal within 10 days of receipt. The following was discussed but not limited to: 1) New rules effective 8/2025, 2) Director’s questions regarding adding diapering and one year old and infants. A copy of the Notice of Inspection Rights was provided at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.

Initial Comments: The following deficiency was observed at the time of the Compliance Inspection conducted on 03.28.25 and are subject to changes pending programmatic review. The Written Plan of Corrections is required to be submitted in the Licensing portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) Items stored in a bathroom are required to be stored inaccessible to children and in either a closed cabinet or on a shelf in a covered, waterproof container, and 2) Effective August 2024, the new DCS portal is required to be used and each facility is required to set up an account for the facility. 3 of 3 Fingerprint cards reviewed were valid via DPS website search.

Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 4/30/2024 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was completed at the time of the inspection. The fingerprint clearance cards for 3 of 3 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: * Ensure children are signed in and out using the correct signature by all authorized individuals. * Ensure staff are signing in and out. * Ensure the outdoor activity area is free of hazards.
