A full inspection was not conducted. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the person in charge at the time of the inspection. Please submit the Plan of Corrections within 10 days of receipt of this Statement of Deficiencies. 3 of 3 Fingerprint Clearance cards reviewed were valid via the DPS website search.

Initial Comments: The following deficiencies were observed at the time of the Initial Inspection conducted on 5/22/2025 and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to a staff at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days. 14 of 14 Fingerprint Clearance cards reviewed were valid via the DPS website search. The following was discussed but not limited to: 1. Children's attendance rosters. 2. DCS submittals must be in each staff file. 3. The correct capacity must be posted in each indoor activity area. 4. Toilet rooms must contain soap. 5. The Statement of Services must be complete with all of the 18 required items. 6. Hand sanitizer must be inaccessible to enrolled children. 7. Flooring must be maintained in a clean condition. 8. Items must not be stacked on top of each other. 9. The portable pools are not permitted to be used. 10. The landscape fabric must be covered and not pose a tripping hazard. 11. Fans must be mounted and inaccessible to enrolled children. 12. Cords must be inaccessible to enrolled children. 13. The temperature in the freezer must be maintained at 0 degrees F or below.
