All equipment and furnishings were not in good repair. While observing in Space 9, I observed a wall mounted pencil sharpening without it's cover; I was informed the cover has been missing for awhile.

Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The 4 feet chain link fencing between playgrounds is not flush at the top and the chain link above the rail is considered a protrusion.

Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. During the walk-through it was also observed that children were being served fish sticks, broccoli/carrots mix, assorted fruits (peaches, peaches/pear mix, pears) and milk for lunch but the posted menu in one classroom reflected fish sticks, broccoli, peaches and milk while the menu posted near the kitchen reflected fish sticks, broccoli/carrots mix, peaches/pears mix and milk for lunch.
