Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Space 5 a parent has given permission to administer Children's Allergy relief medication to her child, 5 years of age. The bottles instructions states do not use unless directed by a doctor for children between the ages of 2-5 years. No doctor authorization was given for either medication.

The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff hired 4-17-23 provided a training certificate dated 11-7-21. Because she completed the training more than a year prior to employment it could not be counted to meet the requirement, she should have completed the training within 90 days of hire.

Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Space 1, four (4) outlets were observed uncovered.
