| Findings: There were 3 infants/toddlers enrolled in care and they were present at the time of the inspection. Also, there is another toddler child who was on the attendance sign-in sheet in the month of September 2025, who reportedly does not attend on a regular basis. The Licensing Specialist was unaware of this child being enrolled until this child was found on the attendance sign-in sheet. This center is not approved to care for infants or toddlers. | Findings: Located on the enclosed play ground was a small plastic play house that did not sit level to the ground. It sat at a slanted position and was not stable. The concern is that the play house could fall over while children are playing near, around or inside of this equipment. Also located on the play ground was a mini trampoline with a handle. The netting on this trampoline was dry rotted and the trampoline was tattered. The director was informed that there is to be no use of a trampoline nor can it be accessible to the child care children. The director stated that the children do not use this trampoline and that she continues to put this trampoline outside of the play ground area and the church administrators put it back inside of the fenced area. It was recommended that the director observe the play ground before the start of child care to ensure that there are no hazards located in this enclosed area. It was also advised that the director communicate with the church administrators about what equipment can and cannot be used on the playground for her child care program. | |||

| Findings: There are two doors to the child care room. Both of these entry/exit doors lead to a hallway. During today's inspection and as noted by the director, there are often events, meetings etc. that occur at this church in other rooms during child care hours. At today's inspection, there were two separate rooms with individuals conducting meetings. Located in the child care room is a door that leads to the hallway where the children's bathroom is located. This door was opened at arrival and for a period of time during the inspection. The director eventually shut this door. It is imperative that this door remain closed at all times while children are in care so that a child does not leave the room. There are multiple visitors who are in the church during child care hours and it is imperative to ensure the safety and security of each child at all times. | |||

| Findings: As the Licensing Specialist was entering the child care room, the director immediately informed the Specialist that she had babies in care. The director indicated that 3 of the 7 children present, were under the age of two. Their ages were 12 months, 14 months & 18 months of age. At arrival the director placed the 3 infants/toddlers in three separate high chairs. Eventually, two of these children began crying and cried for a period of time. The director gave these children a few cherrios on their high chair tray & their sippy cup. The children ate the cherrios & two of them eventually started crying again. The environment was chaotic. The provider was extremenly emotional & cried @ times throughout the inspection. The provider called the aide from her phone to see when she would be arriving. It was apparent that it was unmanagelble for the director to be able to provide care alone with crying infants/toddlers, plus attending to the needs of the other 4 children that were present. While the infant children were crying while sitting in their high chairs, the director reached out to the caretakers of these children and asked for these children to be picked up.
