| Findings: The center self reported an incident that occurred on 1/24/2024. The school age teacher returned to the center after the morning van runs and parked the van in the Salvation Army parking lot across the street. He returned to the building and was cleaning the school age room. Approximately 10-15 minutes later, a woman arrived at the center with an 8 year old child she said was about to cross the street after exiting the center van. The child said she had fallen asleep with her feet on the van chair. The school age teacher had marked the child on the attendance sheet as being dropped off at school. The child resembles another child in care and he may have mistakenly marked her off. He failed to do an in the van check and only checked in the vans windows while walking around it to ensure the van was empty. The school age teacher says he did not see the child on the van when he checked. The owner stated the vans are newer and have higher backed seats which requires a more detailed check of the seats. The child's mother was notified and picked her up at the center around 12:30 PM. The school age teacher has been placed on administrative leave during the investigation. | |||
