Staff person stated that she had no idea where that came from during the interview by the inspector. This facility also, has no knowledge of the origin of the form. Staff person # 1 has had a new health assessment. To avoid any further confusion, we will ask them to fax or email the form, since it is not uniform across the health professional fields. If health assessment form can not faxed or e-mailed directly from the physician's office then only an original health assessment form will be accepted from staff.

I have used this provider for more than 6 months

I HAVE MANY GOOD THINGS TO SAY ABOUT THIS DAY CARE...THE OWNER IS VERY PROFESSIONAL, UNDERSTANDING, LOVING, AND CARING. HER STAFF IS THE SAME WAY. I APPRECIATE THERE HARD WORK THEY PUT IN FOR THE KIDS. THANK YOU MUIR LITTLE SCHOLARS.
