Description: Open since August 2004, Middle Fork Elementary School strives to be a model school in which:

A safe indoor and outdoor environment was not provided for the children. On the playground, eleven metal border ties surrounding the mulched area of the playground were observed to be rusting. Greater than five bolts on/surrounding the gate of the fencing were observed to be extremely rusted and need to be replaced. A small wasp nest, with a wasp crawling just outside the nest, was observed inside and at the top of the stationary structure with the green, cone-shaped roof on the mulched area of the playground. This was removed by a custodial staff member during the visit. A small bottle of lotion with warnings listed and a small bottle of hand sanitizing spray were observed stored less than five feet from the ground and accessible to children. There is a cracked plastic bridge on the stationary equipment that needs to be repaired or removed. There is not six inches of mulch around stationary equipment. There are three pieces of sharp roots sticking out of the ground that need to be removed. There is a piece of PVC pipe sticking up out of the ground that needs to be removed. |
