Noncompliance Area: Review of files reveal that Staff # 1 did not have their educational verification on file at the facility at the time of the renewal inspection. Staff # 3 did not have their experience in childcare documented on file at the time of the renewal inspection.

Correction Required: A facility person's record shall include verification of child care experience, education and training prior to service at the facility.A facility person's record shall include acceptable verification of experience, education or training is a transcript or a diploma or a letter signed by a representative of the experiential, educational or training entity.

Noncompliance Area: Review of files reveal that Staff #3 did not have their emergency plan training on file at the facility for the current renewal.

Correction Required: The operator shall assure that each facility person receives training regarding the emergency plan at the time of initial employment, on an annual basis and at the time of each plan update. The operator shall document the date of each training and the names of all facility persons who received the training and kept on file at the facility.

Noncompliance Area: Review of staff files reveal that Staff #1 and Staff #2 did not have their State Criminal results on file at the time of the renewal inspection.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). SAFETY PLAN: The operator is required to submit to the Children and Youth Agency a plan of supervision or alternate arrangement as required by the Child Protective Services Law. PROVISIONAL HIRE UNSUPERVISED: A provisional employee may not be permitted to work alone with children and must work within the vicinity of a permanent employee.A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL.

Noncompliance Area: There were five children in the rear room with one staff during a period of ten minutes at the onset of the renewal inspection. Of the five children, there was one infant and four older toddlers. Provider

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff:child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The required staff:child ratios must be maintained at all times.

Noncompliance Area: There were two stained ceiling tiles present in the rear childcare room at facility during the renewal inspection.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: 1 outlet cover was not present in the front room used for circle time at the time of the unannounced inspection.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: Upon arrival for the first observation at the time of the most previous renewal inspection, there were 4 children napping in the infant room and the staff was not present for a period of 2 minutes. The staff was observed coming out of the kitchen area. A second staff sat in the front child care room and held a baby while supervising the children in that room. There was also another child in a separate room where Ocdel rep was informed that class instruction takes place. This room was directly behind the front office area where the Director was and the child napped by himself while the lights were out. Provider submitted supervision training verification for Staff #2 yet the verification was not received for the remaining staff- Staff #3.

Correction Required: When children are grouped in similar age levels, the following maximum child group sizes and ratios of staff persons apply: Infants 1:4 with a maximum group size of 8; Young toddlers 1:5 with a maximum group size of 10; Older toddlers 1:6 with a maximum group size of 12; Preschool children 1:10 with a maximum group size of 20; Young school-age children 1:12 with a maximum group size of 24; Older school-age children 1:15 with a maximum group size of 30. IF USING TIERED LIS: The required staff:child ratios must be maintained at all times.Children on the facility premises and on facility excursions off the premises shall be supervised by a staff person at all times. Outdoor play space used by the facility is considered part of the facility premises. TIERED LIS: Children must be supervised at all times. The legal entity must arrange for all facility staff to receive a minimum of two hours of training regarding supervision of children.

Noncompliance Area: The actual date was not present on the log and instead the month and year was listed on the fire drill log at the time of the renewal and there was no location noted of the hypothetical fire at the time of the renewal inspection. Provider submitted corrections and there was no number to determine number of children that evacuated at the time of the most recent fire drill.

Correction Required: A written record shall be kept of the date, the time of day, the hypothetical location of the fire, the evacuation time, the names of facility persons and the number of children participating in the fire drill.
