Noncompliance Area: During renewal inspection certification rep observed the sink dirty downstairs in the school age room. The blinds in the infant room and the room next to the office were broken.

Correction Required: Toys, play equipment and other indoor and outdoor equipment used by the children shall be clean, in good repair and free from rough edges, sharp corners, pinch and crush points, splinters and exposed bolts.

Description: Clean, age appropriate

Noncompliance Area: The playpens in in the infant room were not labeled with the infants name. A mat chart was missing in the preschool room.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Regulation: 3270.102(c)

Description: Outdoor equip.- protective surfacing

Noncompliance Area: During the unannounced visit, certification representative observed no protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The facility had embedded equipment in their play space which was embedded in mulch. The equipment was not mounted over enough mulch to meet the requirements of the USCPSC, the mulch was less than 6 inches in depth.

Correction Required: Outdoor equipment that requires embedded mounting must be mounted over a loose-fill or unitary playground protective surface covering that meets the recommendations of the United States Consumer Product Safety Commission. The equipment must be anchored firmly and be in good repair.

Noncompliance Area: Staff person #-1- was observed to be caring for children unsupervised. Staff person #-1-- has not completed the following pre-service training required prior to caring for children without an AGS/ GS supervision. The staff that was working with Staff 1 is an aide.

Correction Required: A facility shall be operated in conformity with applicable Federal and State laws and regulations. State agencies whose regulations may relate to the operation of a facility include the Department of Environmental Resources, the Department of Labor and Industry, the Department of Health, the Department of Education and the Department of Transportation. Conditions at the facility may not pose a threat to the health or safety of the children. Until such time as the required pre-service trainings are completed, staff person #-1- must be supervised, when interacting with children, by an (AGS, primary staff person, or family child care home staff person) who has completed the required training related to this citation. If there are no staff person(s) available to supervise staff person #-1-, staff person #-1- may not work in a child-care position at the facility.

Noncompliance Area: A young school aged child sat on the floor in the front of the vehicle between a staff and another child while in transport.

Correction Required: A child 7 years of age or younger shall be transported in accordance with the requirements for parents and guardians as stated in 75 Pa.C.S. 4581 (relating to restraint systems).

Noncompliance Area: Children were transported in a 15 passenger van on the morning of 2/20/20.

Correction Required: In accordance with the requirements of 67 Pa. Code Chapter 171 (relating to school buses and school vehicles), the facility may not transport a child in an 11-15 passenger van.

Noncompliance Area: There was a mixture of young toddlers and young school age children on 1/15/2020 per complaint allegation. On 1/27/2020, Ocdel rep witnessed upon arrival that there were five children in the infant room upon arrival to the facility. Of those children, there were 4 infants; one toddler and one staff.

Correction Required: When children are grouped in mixed age levels, the age of the youngest child in the group determines the staff: child ratio and maximum group size in accordance with the requirements in 3270.51 (relating to similar age level). The required staff: child ratios must be maintained at all times.

Noncompliance Area: Per complaint allegation, there was ratio non compliance while toddlers and young school age children were combined on 1/15/2020. Upon arrival to facility on 1/27/2020, there was ratio non compliance of mixed children.

Correction Required: The related or foster children of an operator and the children or foster children of a staff person shall be counted in the staff: child ratio requirements.

Noncompliance Area: High chairs were severely soiled at the time of the unannounced monitoring that occurred on 1/27/2020.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.

Noncompliance Area: Staff #1 and Staff #2 were supervising children in the infant room on the day of inspection. Staff had previously been informed that staff was indicated.

Noncompliance Area: Certification supervisor observed an electrical power strip on the sink in the infant room, while staff washed an infants hands after diapering.

Correction Required: Conditions at the facility may not pose a threat to the health or safety of the children.

Noncompliance Area: Certification Supervisor observed cleaning materials on the counter where food was being prepared for lunch.

Correction Required: Cleaning materials and other toxic materials shall be used in a way that does not contaminate play surfaces, food, food preparation areas and does not constitute a hazard to the children.

Noncompliance Area: Sink in Pre-school room was clogged and inoperable. Lower level restroom was being used for storage and not clean and in good repair.

Correction Required: Toilet areas and fixtures shall be cleaned daily and be in good repair.

Noncompliance Area: Lower level bathrooms did not have hand washing sign's posted at each sink and toilet.

Correction Required: A facility person and an able child shall wash his hands after toileting and before eating. A sign on which this requirement is written shall be posted at each toilet, training chair, diapering area and sink in the facility.

Noncompliance Area: An infant received an injury while in care in which the staff had no account of how the injury occurred.

Correction Required: The legal entity must submit documentation in a format approved by DHS proving that the facility procedure for counting children is followed at all times e.g., a checklist. The legal entity will confirm continuing compliance by sending copies of the documentation to the Regional Office of Child Development and Early Learning on a weekly basis for a period of three months after DHS approves the documentation format.

Noncompliance Area: ON DECEMBER 3, 2018 DURING THE RENEWAL INSPECTION, THE EMERGENCY CONTACT FORM OF CHILD #1 DID NOT CONTAIN SIGNED PARENTAL CONSENT FOR ADMINISTRATION OF MINOR FIRST AID.

Correction Required: A child's record shall contain signed parental consent for administration of minor first-aid procedures by facility staff. Written consent is required prior to admission.

STAFF PERSON #2 ACKNOWLEDGED THAT HE WAS RESPONSIBLE FOR THE 4 YOUNGEST CHILDREN IN THE YARD, WHO WERE ALL PLAYING IN AN AREA WITHIN CLOSE PROXIMITY TO HIM. STAFF PERSON #3 AND #4 WERE RESPONSIBLE FOR THE REMAINING 22 CHILDREN ON THE YARD. STAFF PERSON #1 ACKNOWLEDGED THAT THE AGE OF THE YOUNGEST CHILD IN THE REMAINING GROUP OF 22 WAS 2 YEARS OF AGE.

Correction Required: THE LEGAL ENTITY MUST ARRANGE FOR ALL FACILITY STAFF TO RECEIVE A MINIMUM OF TWO HOURS OF TRAINING REGARDING SUPERVISION OF CHILDREN WITH AN EMPHASIS ON STAFF:CHILD RATIOS THROUGH THE REGIONAL KEY. SE REGIONAL OFFICE OF OCDEL WILL MAKE THE REFERRAL TO THE REGIONAL KEY.

Noncompliance Area: ON AUGUST 31, 2017 DURING THE ALLOCATED UNANNOUNCED INSPECTION,UPON ENTERING THE FACILITY STAFF #1 WAS INITIALLY OBSERVED IN THE LUNCHROOM AREA PREPARING LUNCH; 11 CHILDREN WERE THEN OBSERVED ALONE IN THE INFANT CLASSROOM UNSUPERVISED, STAFF #2 CAME FROM THE BACK PRESCHOOL ROOM TO HELP STAFF #1 COVER THE INFANT CLASSROOM. STAFF #1 ACKNOWLEDEGED THAT THE YOUNGEST CHILD PRESENT IN THE CLASSROOM WAS 4 MONTHS OF AGE AND THE OLDEST CHILD WAS 12 YHEARS OF AGE. TWO SCHOOL AGE CHILDREN THAT WERE PRESENT IN THE INFANT ROOM WERE SENT TO THE BACK PRESCHOOL CLASSROOM. 39 CHILDREN WERE OBSERVED IN THE BACK PRESCHOOL CLASSROOM WITH STAFF #3 AND STAFF #4. STAFF #1 ACKNOWLEDGED THAT THE YOUNGEST CHILD PRESENT IN THE PRESCHOOL CLASSROOM WAS TWO YEARS OF AGE AND THE OLDEST WAS 12 YEARS OF AGE.

Correction Required: An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.

Correction Required: At least two facility persons shall be present in the facility when two or more children are in care. At a minimum, one of the facility persons shall be a staff person.

Noncompliance Area: ON JANUARY 9, 2017 DURING THE RENEWAL INSPECTION,STAFF #1 DID NOT USE SOAP AND WATER TO WASH HANDS AFTER DIAPER CHANGES, SHE USED HANS SANITIZER.

Correction Required: A staff person shall ensure that a child's hands are washed before meals and snacks, after toileting and after being diapered.

Noncompliance Area: ON JANUARY 9, 2017 DURING THE RENEWAL INSPECTION, THERE WAS A PROTECTIVE RECEPTACLE COVER MISSING FROM THE ELECTRICAL OUTLET IN THE TODDLER ROOM NEAR THE CHANGING TABLE.

Correction Required: Protective receptacle covers shall be placed in electrical outlets accessible to children 5 years of age or younger.

Noncompliance Area: ON JANUARY 9, 2017 DURING THE RENEWAL INSPECTION, THE FIRST AID KIT IN THE PRESCHOOL ROOM WAS MISSING SOAP, SCISSORS AND TWEEZERS. THE FIRST AID KIT IN THE TODDLER ROOM WAS MISSING SOAP.

Correction Required: A first-aid kit must contain the following: soap, an assortment of adhesive bandages, sterile gauze pads, tweezers, tape, scissors and disposable, nonporous gloves.
