One of Ms. Ferguson’s sons had his 16th birthday on 10/20/2023, and the required CBC documentation (per ABCMS) was submitted on 11/20/23.

The Emergency Preparedness and Response Plan review for Mr. Ferguson was not on file for review. The Health Questionnaire and Emergency Medical Care Plan review were updated during my visit.

Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. One child enrolled did not have a time of arrival recorded on the attendance sheet.

Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. The outdoor learning environment contained one metal dump truck which was observed to be rusting. In the bathroom accessed by children, a box of fabric softener dryer sheets, one gallon of bleach, a bottle of disinfectant spray, and a box of laundry detergent labeled Keep Out of Reach of Children with additional warnings were observed in stored greater than five feet from the ground, but not locked storage. In the space designated for child care, two individual sized bottles of hand sanitizer were observed greater than five feet up, but in unlocked storage.
