There was an insufficient amount and variety of toys and equipment in the outdoor play environment based on the age of the enrolled children and the capacity of children the facility serves.

In the outdoor environment there were broken toys in the toy box, the fence is loose and tilted over (less than 4 feet due to it falling over), a rug is covering a section of asphalt that has two areas that are tripping hazards in need of repair.
