Findings: Per Allegation Based on the Surveyor's observation and interview with the staff, it was determined that on 11/17/2021, Child #1 and Child #2 were not supervised by Staff #1 and Staff #2 on the playground. Staff #1 was on the porch by a desk and Staff #2 was standing with her back to Child #1 and Child #2 who were next to the slide. Child #1 had their pants down all the way to their ankles and Child #2 was touching Child #1 on their bottom. Both staff were not accountable for the children's actions.
