Findings: The Surveyor found that Staff 1 and Staff 2 failed to apply for a fingerprint clearance card within seven working days of employment. a) The application for Staff 1, start date 12.6.18, was not received by DPS until 1.16.19. b) The application for Staff 2, start date 2.11.19, was not received by DPS until 3.20.19.

Findings: Based on interview, the Surveyor found Staff 1 and Staff 2 worked together with enrolled children. However, neither staff was qualified as a teacher caregiver and they were not supervised by a staff member that was qualified as a teacher caregiver.
