Initial Comments: No deficiencies were observed at the time of the Initial Monitoring Inspection conducted on 11/5/2025 and is subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The following was discussed but not limited to: 1). Ensuring that personal items are labeled with the child's first and last name.
INSP-0158907 2025-09-10 Compliance (Initial) Complete
Initial Comments: The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 9/10/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: 1). Ensuring that the evacuation map is posted in all classrooms. 2). Ensuring that the first aid kit has re-closable zip-lock gallon bags. 3). Ensuring that all fall zones have the required 6 inches of resilient surfacing. 4). Ensuring that medication authorization forms are fully completed before administering medication. 5). Ensuring that the diaper-changing area is kept clear of items not used for diapering. 6). Ensuring that brooms and rakes are not accessible to enrolled children. The outside bathroom, facing 75th Ave and the school's parking lot, were discussed and a barrier was recommended for safety/protection and supervision purposes.
