Noncompliance Area: DURING THE RENEWAL INSPECTION THE WRITTEN COVID-19 HEALTH AND SAFETY PLAN WAS INCOMPLETE. THE POLICY DID NOT ADDRESS THE REQUIRED AREAS TO ALIGN WITH CDC GUIDANCE AND MINIMIZE THE RISK OF COVID-19. THE PROVIDER DID NOT HAVE A CURRENT FIRE DETECTION SYSTEM INSPECTION REPORT.

Correction Required: Staff persons shall participate, at least annually, in firesafety training conducted by a fire protection professional. Staff persons and volunteers shall receive training in maintenance of smoke detectors, the duties of facility persons during a fire drill and during a fire and the use of the facility's fire extinguishers, not including discharge of the fire suppressant agent. A facility person's record shall include verification of child care experience, education and training following the outset of service at the facility.

Noncompliance Area: DURING THE RENEWAL INSPECTION THE FILE OF STAFF 1 (HIRED 9/1996) AND STAFF 2 HIRED (4/12/17) DID NOT INCLUDE AN APPLICATION FOR OR COMPLETED NSOR CERTIFICATE.

Correction Required: The operator shall comply with the CPSL and with Chapter 3490 (relating to protective services). A facility person's record shall include a copy of requests for the criminal history record and child abuse registry clearance information, a copy of the disclosure statement and a copy of the completed clearance information required under the CPSL. Facility Person 1 AND 2 may not work in a child care position at the facility.

Noncompliance Area: ON 02/26/2020, THE CERTIFICATION REPRESENTATIVE (CR) ARRIVED APPROXIMATELY AT 11:15 AM AT THE FACILTIY IN AN ATTEMPT TO CONDUCT AN UNANNOUNCED INSPECTION BUT WAS NOT GRANTED ACCESS. AFTER RINGING THE BELL AND THEN KNOCKING ON THE DOOR, A FEMALE INDIVIDUAL OPENED THE DOOR. CR COULD HEAR CHILDREN CRYING FROM INSIDE OF THE FACILITY. CR ASKED FOR THE OWNER OR DIRECTOR. HOWEVER, THE WOMAN INFORMED THE CR THAT THE OWNER WAS NOT AT THE FACILITY. BEFORE THE WOMAN COULD CLOSE THE DOOR, THE CERTIFICATION REPRESENTATIVE PRESENTED HER WORK ID BADGE AND INFORMED THE WOMAN THAT SHE WAS THERE FROM THE DEPARTMENT OF HUMAN SERVICES WITH THE PURPOSE OF CONDUCTING AN UNANNOUNCED INSPECTION THAT DAY. THE WOMAN STATED THAT SHE NEEDED TO CALL THE OWNER AND CLOSED THE DOOR. WHILE WAITING THE CR MADE ATTEMPTS TO REACH THE OWNER BY CALLING THE FACILITY PHONE NUMBER AND THE LISTED CELL PHONE NUMBER. CR CONTINUED TO RING THE BELL AND KNOCK ON THE DOOR. THE WOMAN NEVER RETURNED TO THE DOOR. CR LEFT FACILITY AT APPROXIMATELY 11:45 AM.

Correction Required: A staff person shall provide to agents of the Department immediate access to the facility, the children and the files and records

Noncompliance Area: During the renewal inspection the cots that were observed were not labeled for use by specific children. The chart identifying the children who were using the cots did not correspond with the actual children who were receiving care.

Correction Required: Individual, clean, age-appropriate rest equipment shall be provided for preschool, toddler and infant children as agreed between the child's parent and the operator. The rest equipment shall be labeled for the use of a specific child and used only by the specified child.

Noncompliance Area: During the renewal inspection the file for staff person #1 was reviewed and it did not contain a health assessment verifying suitability to work, and freedom from communicable tuberculosis.

Correction Required: A facility person providing direct care who comes into contact with the children or who works with food preparation shall have a health assessment conducted within 12 months prior to providing initial service in a child care setting and every 24 months thereafter. A health assessment is valid for 24 months following the date of signature, if the person does not contract a communicable disease or develop a medical problem.A facility person's record shall include a written report of initial and subsequent health assessments, including the results of initial and subsequent tuberculin skin tests, x-rays or other medical documentation necessary to confirm freedom from communicable tuberculosis.

Noncompliance Area: During the renewal inspection there was no menu posted at the facility.

Correction Required: An operator shall conspicuously post the menu at least 1 week in advance or provide a menu to each family.

Noncompliance Area: During the renewal inspection an infant bottle was observed that had not been labeled with the name of the child it was intended for.

Correction Required: Disposable nursers and bottles shall be labeled with the child's name.

Noncompliance Area: During the renewal inspection the operator was not able to produce the letter that is to be sent annually to the local authorities notifying them of the facility's use of pedestrian and vehicular routes around the child care facility.

Correction Required: An operator shall notify local traffic safety authorities annually in writing of the location of the facility and the program's use of pedestrian and vehicular routes around the day care facility.

Noncompliance Area: During renewal inspection on 6/21/18, while doing a walk through of the facility, Cert rep observed peeling paint near the entrance on the inside of the facility near front door, near the cots and next tot he refrigerator.

Correction Required: Floors, walls, ceilings and other surfaces, including the facility's outdoor play space surfaces shall be kept clean, in good repair and free from visible hazards.Peeled or damaged paint or damaged plaster is not permitted on indoor or outdoor surfaces in the child care facility.
