Initial Comments: The following deficiencies were observed at the time of the Compliance Inspection conducted on 6/10/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. Please submit a Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Survey was emailed following the inspection. Please complete it within 10 days of receipt. The fingerprint clearance cards for 4 of 4 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *Ensure supervision of children at all times.

Initial Comments: There were no deficiencies observed at the time of the annual Compliance inspection conducted on June 12, 2024. Compliance Officer #1: Celeste Angulo Compliance Officer #2: Pat Morgan-Martinez 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. A link to the Empower survey was emailed to the facility director. The following items were discussed but not limited to: *Staff have 30 days to get Fingerprint Clearance Card after 18 yrs-of-age. *Direct Central Registry Affidavit proof of submittal.

Initial Comments: The following deficiencies were observed at the time of the Compliance inspection conducted on 06/20/2023, subject to changes pending programmatic review. Please submit via the Licensing portal within 10 days of receipt of the Statement of Deficiencies. 5 of 5 fingerprint clearance cards were valid via a DPS website search.
