Description: The 2013-2014 academic year is well under way at the Early Childhood Learning Center. With our recent delivery of items from Lakeshore Learning, the brand new floors and cabinetry and the brightly painted walls, the center looks amazing! Our staff works hard to develop clientuploads/ECLC/IMG_1474 (300x225).jpgchallenging activities for your children so that they can learn and progress at their own level. With the use of guidelines from the National Association for the Education of Young Children, NAEYC, the premier accrediting organization and the Arizona Early Learning Standards, our teachers feel confident that they will meet the individual needs of each and every child at our center.

Findings: Per allegation Based on Surveyor's observation, review of facility documentation and information provided by the staff at the time of the complaint investigation, it was determined that the bottles were sometimes propped for feeding infants. The facility Staff stated that "sometimes when there are several children crying at the same time and need to be fed, the infants are put in the high chair and their bottles are propped up". An additional staff stated that "although she does not prop the infants, she has witnessed other staff members do it".

Findings: Per allegation Based on Surveyor's observation, review of facility documentation and information provided by the staff at the time of the complaint investigation, it was determined that the Fingerprint clearance card application for Staff #1 (start date: 5/29/2018), had a status of "No Application found" when the Fingerprint Clearance card application number was submitted to the DPS fingerprinting website. It was also determined that DPS did not receive the Fingerprint Clearance card application for Staff #2 (start date: 5/8/2018) until 7/2/2018. The Fingerprint Clearance card for Staff #3 (start date: 10/14/2016) expired on 6/25/2018 and DPS did not receive the Fingerprint Clearance card application until 7/2/2018.

Findings: Based on facility documentation and the Surveyors' observations, it was determined that 12 out of 30 enrolled children's Emergency, Information, and Immunization Record cards were incomplete and lacked the following required information: Child #1 and Child #7: The name and number of the second emergency contact and the name and number if the health care provider. Child #2: The home address for the child's parents and the name and number of the second emergency contact. Child #3: The home address for the child's parents, the name of the individual to be contacted in case of injury or sudden illness of the child and the name and number of the health care provider. Child #4, Child #5 and Child #6: The date of enrollment. Child #8, Child #10 and Child #11: The name and number of 2 emergency contacts. Child #9: The name of the individual to be contacted in case of injury or sudden illness of the child and the number of the health care provider. Child #12: The date of enrollment and the name and number of the second emergency contact.

Room 23: There were 9 children in the classroom but the roster reflected 10.

Findings: Based on Surveyor's observation, review of facility documentation and information provided by the staff at the time of the inspection, it was determined that the file for Staff #3 lacked documentation to qualify the staff as a teacher care-giver. Upon the Surveyor's arrival at the facility Staff #3 was seen to be transporting a group of children on the bus for a field-trip.

Staff #3 (start date: unknown): A start date, the name and address or an emergency contact, statement of immunity and copy of a High school diploma or equivalency.
