Findings: After a review of 6 staff files, it was determined the following was lacking: Staff #1 (start date 12/13/2019) had a fingerprint clearance card with an issue date of 10/17/2016, prior to employment at the facility. There was no complete documentation to show DPS had been contacted to verify the status of the clearance card, as required.

Findings: Upon arrival into the Toddler 1 and Toddler 2 rooms (rooms are divided by a 1/2 wall), the temperature was measured to be 64F and 64.7F, respectively. There were enrolled children present in Toddler 2 room at this time.

Findings: In the Head Start 2 room, the temperature of the water from the diaper-handwashing sink was measured to be 70F, after running for several minutes. Staff in the room stated the water does not get warm. In the Music/Movement room, the diapering station was observed next to a sink which had the water turned off and had a drinking fountain spigot attached to the sink. Staff in the room stated they use the other sink which is next to the first sink closest to the diapering station. This second sink was not next to the diapering station, as required.

Findings: After a review of 8 staff files, it was determined the following was lacking: Staff #1 (start date 3/12/2018) only had a copy of a GED certificate and not the score sheet or other form of documentation from the Department of Education to verify the certificate was valid.

Please be careful with an employee at this daycare center her name is Rosa Arellano she sleeps around with the dads of the kids at attend this daycare center. I found alot of texts and naked pictures of her on my sons fathers phone. Be aware for the safety of your children
