| Initial Comments: The following deficiencies were observed at the time of the Initial Compliance inspection, conducted on 7/23/2026, subject to change pending programmatic review. The plan of correction is due within 10 days of receipt of this Statement of Deficiencies. NOTE: The license will be issued upon confirmed corrections of all deficiencies. Twelve staff files were reviewed, and 12 of the 12 fingerprint clearance cards were verified as valid via the DPS website. The following was discussed, but not limited to: *The Infant Room diaper changing sink must be maintained in a clean and sanitized condition, including behind the sink faucet and at the base of the fall ledges. *Restroom sink faucets must be maintained in a clean condition. *Designated exits must be unlocked and unobstructed during the hours of operation, including the barrier gates between indoor activity areas. *The playgrounds must be maintained free from hazards, including protruding underlayment. *Playground equipment must be maintained in a clean condition. *Please send the manufacturer's specifications for the playground surfacing mats, or replace with at least 6" of resilient surfacing/woodchips. | |||
