Initial Comments: The following deficiencies were found at the time of the Initial Licensure Inspection conducted 03/27/2026, subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The facility was not licensed at the time of the inspection. Items discussed, but not limited to, were as follows: -Walk-through and drive-through gates must remain closed when children onsite -Lesson plans. activity schedules, capacity, etc. must be posted inside activity area -Plungers and toilet brushes must remain inaccessible -Supervision and ratio must be maintained for restroom rotations -Immediate access to facility must be granted -Fingerprinting and background check timeframes -Signing in and out for children and staff -Children’s Emergency, Information, and Immunization Record card completion -Staff: files, start date, and training (10-Day and Annual) -Discipline and guidance procedures -Children with Special Needs: Individualized Plan -Mandated Reporting of suspected abuse and neglect -Medication (form, storage, labels, rescue meds, etc.) -Supervision (nap, outside, and inside) -Field trips: Anytime leave licensed area -Fire safety: scheduled annual inspections with Fire Marshal or local Fire Department, extinguishers yearly and fire/emergency drills monthly -Contact Compliance Officer when any changes are being made to the facility, Director position, or facility closure on a normally scheduled day Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies.
