Findings: Upon review of the facility's sign in/out records, Surveyor 1 noted the following; -Child 1 was signed in on 8/16/19. The inspection took place on 8/15/19, a day prior to the documented sign in day. Child 1 was signed out at 1:55 pm on 8/15/19, however, Surveyor 1 reviewed the sign in/out documentation on 8/15/19 at 1:32 pm approximately 23 minutes prior to the documented sign out time. The Director reported that Child 1 was present in the classroom and parents had inadvertently signed out on the wrong day. -Child 2 was signed in on 8/15/19 with a first name and last initial; -Child 3 was signed in on 8/13/19 on the signature line with quotation marks; -the time was not entered when Child 4 was signed in on 8/15/19.

Findings: Upon review of classroom 19's roster documentation, Surveyor 1 noted that there were 15 children present in the classroom. The classroom roster listed 11 children as being present in the classroom. The Director reported that some children who ride the bus had not been signed in. The Director also reported that attendance is also documented online and submitted to the school office, however, when the electronic roster was accessed and shown to Surveyor 1, there were 14 children listed as being present.

Findings: Upon inspection of the indoor activity areas, the Director reported that a mat, sheet and blanket were not being kept at the facility when Surveyor 1 asked if they were kept on site.
