Initial Comments: The following deficiency was observed at the time of the Compliance Inspection conducted on 2/10/26 and is subject to changes pending programmatic review. A full inspection was conducted. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection.

Initial Comments: The following deficiencies were found at the time of the compliance inspection conducted on 2/24/25, and are subject to changes pending programmatic review. 2 of 2 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The Empower Survey link was emailed to the facility. The DES group size was observed at the time of the inspection. Please complete the Plan of Corrections via the online portal within 10 days of receipt of the Statement of Deficiencies.

Initial Comments: The following deficiencies were found at the time of the annual Compliance Inspection conducted on February 26, 2024, subject to changes pending programmatic review. A full inspection was conducted at this time. Three fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Inspection Item Expiration Dates: Insurance: 07/01/24 Fire: 11/16/24 Gas: 07/20/24 Sanitation: 01/31/25 Items discussed, but not limited to: -District substitutes must maintain a complete physical or digital staff file -Restroom storage/Soiled diaper containers must be kept inaccessible to children -Updated Entrance Letter and DCS Direct Service form The Plan of Correction was not accepted at the time of the inspection.
