Description: Sonshine Preschool has been loving children since 1988. We are so proud to serve the community in this outreach ministry of Eastside Covenant Church! We offer preschool children a high quality enriched learning environment, as well as being a Christian-Based program. Our program licensed by the Department of Health Services, is run non-denominationally, and the children enjoy weekly chapel times. Families love having their children here because they really do get the best of both worlds!

Findings: Upon interview with the Director, it was found that a child was brought into the front office for care, however, the front office is not licensed.

Findings: Per Staff interviews, they were recently trained on a new behavior management technique and they used it with Child #3. According to their 2019-2020 Parent Handbook, individual plans are created with the child's Parents, Teacher, and Director. However, documentation of having included the parents of Child #3 in the creation of this new behavior management technique was not on file.

Findings: The Team Leader and Surveyor reviewed thirteen Emergency, Information, and Immunization Record cards and observed the following: - The card for Child #1 did not contain the date of enrollment or the physician's name and telephone number. - The card for Child #2 did not contain the two required emergency contact telephone numbers. - The card for Child #3 did not contain the date of enrollment.

Findings: The Team Leader reviewed the file for Staff #1 (start date 10/24/19) and observed that the new employee 10-day training log was signed by the Director on 1/27/2020, therefore, it was signed outside of the required timeframes.

Findings: The Team Leader reviewed the file for Staff #2 (start date 2/8/17) and observed that it contained 5.5 hours out of the required 18 hours of annual training between 2/8/19-2/8/20.

Findings: Per Allegation Per conversation with the Director, Child #3 took their clothes off and was unwilling to redress themselves. The Director waited alone with the Child for them to choose to put their clothes back on. This process lasted over an hour. Child #3 is non-verbal and wears a diaper.

Findings: Per conversation with the Director, when Child #3 was unwilling to put their clothes back on they remained in the classroom from 4pm-4:45pm. The Director then took Child #3 into her office until 5:15pm. Therefore, Child #3 was separated from their classmates for longer than the three minute maximum. Per interview with the Director, Child #3 had regained composure however, was not putting their clothes back on.

Findings: According to the Director, they do not complete field trip permission forms when they take the children to the ramada area. The ramada area is not a licensed area and a completed field trip permission form would be required.
