Findings: RB does not have evidence of having a second lead test at 2 years of age. The provider shared with the Licensing Specialist that RB has a doctor's appointment scheduled for tomorrow. BD also does not have evidence of having a 2nd lead test at 2 years of age. RM (younger sibling) is missing evidence of his 1st lead test at 1 year of age. Additionally, RM (younger sibling) needs 1 more Hep B vaccination as well as an MMR and Varicella vaccination. LR does not have evidence of having her 3rd Hep B vaccination. The provider shared with the Licensing Specialist that LR's doctor said that she is not able to get her 3rd Hep B vaccination until she is 12 months of age due to a health issue. Please forward a copy of the lead test results for RB, BD, RM (younger sibling) and an updated immunization record for RM (younger sibling) to the OCC.

Findings: Upon review of children's records, it was revealed that child FB does not have Health Inventory Part I or Part II and AC does not have Health Inventory Part I. Child RB and child BD need another DTP immunization for each of them. Please notify the respective parents so they can contact their physicians to obtain the needed documentation. Please provide this documentation to OCC for the correction for this non-compliance. It was also revealed that 2 year old BA and 2 year old AC have no documentation of having had any blood lead testing. They should have had one performed at 12 months old and another at 24 months of age. Children XH and JP, who are also 2 years old, have both had one blood lead testing. Please have them have their 24 month old blood lead testing performed. Notify each parent so they can notify their respective physicians for the need for the testing. Provide results of the blood lead testing to OCC for the correction of the non-compliance.
