Findings: Located in a child's file was a completed Medication Authorization form, which was completed by the child's parent. This form was completed for the use of a non-prescription medication that is intended to be ingested. It was documented that a dose of this medication was given to a child in 2/2021. The medication authorization form was not completed by a licensed health practitioner and was only completed by the parent. When medication is to be orally taken, ensure that this form is completed by a health practitioner that includes the medication and the dosage prior to administrating the medication. This form expired in 2/2022 for this particular medication. The provider will need to obtain an updated form if this medication is to be given again.
