Findings: The two infants need documentation of their 15 month immunizations which includes the 4th DTAP and 4th Hib. Please have their parents obtain the information from the physician and add the documentation to the children's files. One child had the lead blood test done in July 2017 but will also need it done when he turns 2 in October 2018. The other infant is now 17 months and does not have documentation of the lead blood test. If the test was completed at 12 months, please obtain the documentation from the parent. The parent can utilize page 4 of the current health inventory for the physician to document the blood test results. If the child has not had a lead blood test completed, it will need to be done. The physician can decide if they are going to wait until the 2 year old well visit (as this is when the second lead blood test is required).
