Findings: There was an emergency card that was signed on 6/24/22 and has not yet been updated. Please have the parent update this emergency card. It was suggested to have all of the parents update the emergency cards since it is getting close to the annual date as to when they previously signed the form.
2024-03-12 Mandatory Review 13A.15.05.05B Open
Findings: The provider has her backyard fenced in. There is a gate attached to this fence and there is gap which is an entrapment space under this gate. The provider stated she would add another piece of wood to the bottom of this gate in order to prevent a child from getting entrapped. This correction needs to be fixed immediately. The provider's home is located near a very large community pond. The provider agreed to submit a photograph of this correction immediately to the Licensing Specialist.
2024-03-12 Mandatory Review 13A.15.05.06F Corrected
Findings: The provider had a specialized black cover that wrapped tightly around the pack-in-play. This cover also partly covered the top of the pack-in-play and the other half of the pack-in-play was open/exposed. The child was in the pack-in-play at the time of the inspection and he was standing with his head exposed from this opening. The child did not appear to be in any distress. The provider also removed this child from the pack-in-play during the beginning of the inspection so that he could play with the other children. The Licensing Specialist discussed the safety concerns of using this cover. The cover was dark in color and it did not allow for the provider to visibly see through the sides of the cover. The provider would only be able to see this child by looking from the top of the cover where the opening was located. It was also discussed that the there should be air freely moving through the pack-in-play without any type of cover constricting any type of air flow. The provider stated she only used the cover so that the other children would not distract this child during nap time. She immediately removed the cover and agreed to not use this item for child care purposes.
2024-03-12 Mandatory Review 13A.15.06.02B(2) Corrected
Findings: The provider completed this first year training on 7/28/2023. It was supposed to be completed by the end of May 2023. Ensure that continued training is completed within the required timeframes.
2023-04-25 Mandatory Review 13A.15.03.04A(1) Corrected
Findings: At the time of the inspection, the provider was unable to locate three of the emergency cards for enrolled children. The provider was able to provide the names and ages of these children. As discussed at today's visit, ensure that the emergency cards are obtained prior to a child being enrolled in the family child care and that they are always accessible. The provider agreed to submit copies of the emergency cards this week via email to the Licensing Specialist.
2023-04-25 Mandatory Review 13A.15.04.03A Corrected
Findings: The provider's current Certificate of Registration shows that the provider is approved to care for one infant because at the time of when this registration was generated, the provider had an infant of her own. The provider's child turned two years of age on 4/14/23. The provider currently has two children enrolled in her program that are under the age of two. Only one of those enrolled children were present at today's inspection. She indicated that this child started child care last week, which was after her own child turned two. The provider was unable to locate the emergency card of the infant child who was present at the time of the inspection. As discussed, please ensure that the ages and amount of children that you have in your family child care, reflects what is documented on your Certificate of Registration. The Licensing Specialist will update the Certificate of Registration to identify the approval for two children under the age of two. The provider had two pack-in-plays present.
2023-04-25 Mandatory Review 13A.15.05.04B(3) Corrected
