In space #108, the classroom with two year old children, the refrigerator thermometer did not work; the refrigerator was cold, but the thermometer was reading fifty-eight (58) degrees Fahrenheit.

Six (6) staff hired in 2025 had staff medical reports on file that were completed more than twelve (12) months from their dates of hire. A Four- or Five- Star program serving four year old children was not implementing an approved curriculum. The curriculum could not be located during the visit.

A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The child care operator has not notified the Division through the Automated Background Check Management System (ABCMS) portal of the staff that are working at the child care facility.
