All stationary equipment, more than 18 inches high, was not installed over protective surfacing. Surfacing under the slide was at a depth of four inches. Mulch is needed in this area and the other areas need to be fluffed.

Kitchen had Clorox and Fabuloso with double warnings that were not in locked storage. You corrected this during the visit, and we discussed your new chemical dispenser being installed. You are going to speak with Lisa Browning , Environmental Health Inspector , and see what rules to follow for sanitation.

Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Small rocks were accessible in space# 3B with a two-year-old child present. This is a choking hazard.

Individual responsible for a group of children did not meet the preservice requirements for a lead teacher or teacher. A staff member that is in 12th grade was left alone with eleven (11) two and three year old children while the the lead teacher was in the office during nap time.

Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. The date and the provider was not documented on the orientation form for M.F. to indicate completion of orientation within the six topic areas as required.

Each employee's personnel file did not have an Annual Evaluation in her file. You are going to work with a form for staff files. Personnel file did not contain an annual staff evaluation for T.S. and R.H.

Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. T.S. date of hire was 10/19/22. She had not completed health and safety training within one (1) year of hire. Transportation was not completed in this course work. Center staff did not comply with the safe sleep policy. Infant Classroom remained over 78 degrees then an hour later it was 80 degrees. Infants were moved to another room where air-condition.
