|Findings: Based on Surveyor observation of 30 Emergency Information and Immunization Record cards, and a conversation with Staff #1, it was determined 4 cards were missing the following: Child #1: Father's name address and phone number; phone number of the health care provider. Child #9: 1 Emergency contact name and phone number. Child #11: Name and phone number of a health care provider. Child #12: Name and phone number of a health care provider.|
|Findings: Based on Surveyor observation, and a discussion with Staff #1, it was determined the following Emergency Information and Immunization Record cards did not contain the child's immunization record: Child #2,3,4,5,6,7,8,10,11,12.|
|Findings: Based on Surveyor observation, and a discussion with Staff #1, it was determined the surfaces on the diaper changing tables in the infants and the 1's room had chips in the surface making them unsanitizable.|
|Findings: During a review of the children's attendance records and interview with the Director, Surveyor determined that 3 of 31 children's sign in/out records were incomplete. 1) Child #1 was not signed out and lacked a time out on 5/2/2017; 2) Child #2 was not signed out and lacked a time out on 5/24/2017; 3) Child #3 was not signed out and lacked a time out on 5/4/2017.|
|Findings: Based on Surveyor observation and interview with the Director, it was found that (2) walking 1 yr old children were in the Infant room with other infants when there were more than 6 children present at the facility.|
|Findings: The Surveyor observed that there was not water available to the children in the 1's Room.|
|Findings: The Surveyor observed that the lower cabinet lock under the diaper changing table allowed a gap of 1/2 inch between the 2 cabinet doors in the 1's Room.|
