Findings: Based on Surveyor's observation at the time of the inspection, the base of the metal fencing along the eastern side of the playground had become detached from the fence due to rust. The rusted fencing material was lying on the ground below the fence, and had several areas of rust resulting in sharp edges and chipped paint.

Staff #1 has been the Director, however, since August 2018.

Findings: Based on the Surveyors' observations, there was crumbling rubber molding pulling away from the bottom edge of 2 sections of the playground fence that was laying on the ground. There were 4 large metal bolts protruding (4 " ) from the back of the yellow bench near the climber on the playground. The yellow bench was wobbly and tipped to the side when the Surveyors' touched it. There were areas of missing paint on the yellow bench, exposing rough, splintery raw wood.
