Please have the parents obtain the lead blood test documentation from the physician and add to the children's files. Lead blood tests at 12 and 24 months are required for all children born after 1/1/2015. If a lead blood test was not done prior to 24 months of age, only one test is required after 24 months of age. If only one test was done and was completed prior to 24 months of age, then another lead blood test must be completed after 24 months of age.
